Your Appointment

1. Complete the pre-visit registration link sent via text or email. This will allow you to complete all required forms and enter your medical history prior to your visit. This is sent to you 3 days and then again 2 days prior to your appointment. This will help us avoid appointment delays & ensure we have your correct information in our system.

2. New patients – All new patient paperwork will be electronic on your pre-visit registration link. If your registration is complete, please arrive 15 minutes prior to your appointment for check in. If you experience difficulty completing the registration, please arrive 30 minutes early. You will need to bring your insurance card, drivers license and method of payment.

Established patients – If your pre-visit registration is complete, please arrive 10 minutes prior to your appointment or 20 minutes early if you have difficulty completing the registration. Please notify us of any new insurance changes.

3. Obstetric patients: Patients are allowed to have 1 guest accompany the patient to their appointment, including ultrasounds. Please limit additional guests due to lack of space.

4. If you have had any flu like symptoms, cough, shortness of breath, fatigue, fever, GI symptoms, leading up to your appointment, or have been in contact with anyone with these symptoms, please call our office to reschedule your appointment.

Financial Information

All payments are required at the time of service, this includes deductibles and copays. If you are a self-pay patient, payment is due in full at time of service to receive the discount. Please know if we verify benefits on your behalf as a courtesy, it is only an estimate provided by your insurance company and full patient responsibility cannot be determined until a claim is processed. You are responsible to know your own individual benefit coverage.

Our practice has adopted a card on file policy in the event there is a patient responsibility owed after insurance adjudication. We run our payments through our HIPAA compliant card reader and our office staff do not have access to your card. Our system will automatically email you 5 days before charges would occur.
